Fee: Club Membership for the Fall 2010 season is $400 by June 30th, 2010

Refund Policy: Fees are refunded only in the case of an unforeseen medical condition that prohibits the player from participating in our soccer program (i.e. injury). No refunds after August 21st, 2010 (three weeks prior to the first game of the season).

1) Medical Form
Please complete the MSI Club registration confirmation/Medical Authorization.

2) Proof of Age
Please provide a copy of a proof of age document.
The league accepts the following documents:

• Birth Certificate (Foreign documents have to be translated)
• Valid Passport
• Proof of Age card from Motor Vehicles Administration (MVA)
• US Club or USYS player pass (must be from last season)

Remember, this needs to be only a copy.

3) Passport size picture
Please mail a 1" x 1" or a passport size picture of your child to the address below. The picture is used to create the player pass.

4) Submitting the material

Mail all documents with the picture, so that we can obtain the original signed form. Documents can also be handed in personally to our registrar only. Do not hand documents to coaches unless notifying the registrar first.

New players:
Your child will need a team uniform to compete in the upcoming season.
This is a very exciting moment for most players. Please place your order in a timely fashion (ASAP) to ensure the uniform arrives before the season starts.
Due to the volume of orders, it can take up to 10 weeks for uniforms to arrive. On average, they take 4 weeks.
Each player will receive a home (blue) and away (red) jersey with matching shorts and socks. Uniforms are specially ordered with the MSI logo as well as with Toca Juniors Patch. The Cost is $100.
