Fee: Club Membership for the Fall 2009 season is $400.

Refund Policy: Fees are refunded only in the case of an unforeseen medical condition that prohibits the player from participating in our soccer program (i.e. injury). No refunds after September 1st (three weeks prior to the first game of the season).

Payment Schedule: To guarantee a spot on the roster, payments should be made by June 26th, 2009

Payment Method: Payment can be completed in two ways: or Check. accepts all mayor credit cards. Note, it is not necessary to hold a Paypal account to complete the payment. Just click on "don't have a PayPal account?" to proceed without an account.

1) Medical Form
Please complete the MSI Club registration confirmation/Medical Authorization.

2) Passport size picture
Please mail a passport size picture to the address below. The picture is used to create the player pass.

3) Submitting the material: Two steps!

A) E-fax
Once you have filled the Registration form. Please fax it to our E-fax at: 323-313-0990 attn: Toca Juniors FC.

B) Mail all documents with the picture, so that we can obtain the original signed form. Documents can also be handed in personally to our registrar only. Do not hand documents to coaches unless notifying the registrar first.
